INSURANCE

We accept almost all standard insurance
policies. There are some policies that are
not standard insurance and we are not
able to accept these. Please call our
office and we can tell you about your
specific policy. At the time of the
appointment if you have you bring your
insurance card and a blank form, we will
ask you to pay your deductible and an
estimated co-payment, which we will
figure for you. This is only an estimate
and if your insurance company does not
pay as anticipated you will be responsible
for any balance. We have no control over
what your insurance company does
therefore we cannot be responsible for
there actions.
